What’s inside

This editable toolkit helps care teams involve people receiving support in decisions about services, policies and resources. It provides a practical way to plan participation, capture different views and show how input influenced the final decision.

  • A project scope template to clarify which decisions people can influence
  • Guidance for choosing feedback, consultation or co-design
  • An accessibility, consent and participation checklist
  • Session prompts and a record of ideas and differing views
  • A “You said, we did” decision log and change review

How to use it

Use a separate copy for each improvement project. Before inviting people to participate, complete the decision scope and explain when and how you will report back.

  • Ask each person how they would like to contribute and what communication support they need.
  • Use the session record to capture ideas accurately without forcing agreement.
  • Record decisions and reasons in the “You said, we did” log.
  • Test the proposed change with people affected by it.
  • Share an accessible summary of what changed and what happens next.
